How to Apply
The CDI Head Start Program is accepting applications for children between the ages of 3 and 5.
Eligibility for the Head Start Program is based on the Federal Poverty Guidelines. Following the completion and submission of the Online Application or download the PDF Application, please contact the center for which you are applying or the central office to schedule an appointment for an interview.
The following information must be submitted when you interview:

Proof of child's birth (Birth Certificate, Hospital Letter, etc.)

Proof of residency (utility bill, etc.)

Current pay check stubs (4 most recent)

1040 Income Tax Return or W-2's

Social Security Statement (SSA 1099)

Social Security Income (SSI) or TANF documentation

Foster Child documentation, if appropriate

Child Support Documents
-
Unemployment Compensation
